Investigation Summary
On April 26, 1999, Employee #1 sustained a puncture wound to his left foot when he pulled the trigger of an Alkota High Pressure Washer Model E435G while the nozzle of the wand was resting on his foot. Employee #1 was using the pressure washer to clean 15-square-ft metal screens. The machine operated at a maximum pressure of 3500 psi. When Employee #1 pulled the trigger, the water pressure punctured his rubber boots and his foot. Employee #1 stated that he was engaged in "horse play" when this injury occurred.
