Employee Thrown From Vehicle While Spraying Weeds

On September 3, 1998, Employee #1 was spraying weeds using a Randal, model RW-5, spray rig. The vehicle on which Employee #1 was riding had two seats at the front and a 300 gallon container of weed killer behind the seats. Employee #1, along with his coworker, was spraying weed killer in a citrus orchard. As they were riding down an 8 to 11 foot slope the brakes failed. As the vehicle continued down the incline the rear of the cart began to rise off the ground. When this happened, Employee #1 was thrown from his seat to the ground in front of the cart. As the rig continued to travel forward several feet he pushed in front of it. Employee #1 ended up under the front of the vehicle before the vehicle finally stopped.
