Employee fractured skull in fall from tank

At approximately 6:15 p.m. on January 24, 1996, Employee #1 was standing atop a burner tank, repairing an overhead pipeline. He fell over 7-1/2 ft to the floor and suffered a fractured skull. Employee #1 was hospitalized at Antelope Valley Hospital in Lancaster, CA, until January 27, 1996. The burner tank did not have a guardrail, and Employee #1 was not provided with any form of fall protection.
