EMPLOYEE INJURED IN FALL THROUGH HOLE IN ROOF

AT APPROXIMATELY 7:40 A.M. ON AUGUST 23, 1993, EMPLOYEE #1 REMOVED A SECTION OF A PANELED ROOF STRUCTURE TO STRAIGHTEN A BENT METAL STANCHION THAT WAS CREATING A LUMP IN THE RECENTLY COMPLETED ROOF. AS EMPLOYEE #1 APPLIED TORQUE TO A BOLT ON THE HANGER, THE WRENCH SLIPPED, CAUSING EMPLOYEE #1 TO FALL THROUGH A 22-INCH BY 18-INCH HOLE TO THE CONCRETE FLOOR, A DISTANCE OF 26.5 FEET. HE WAS HOSPITALIZED FOR THE RESULTING INJURIES. EMPLOYEE #1 WAS NOT WEARING HIS SAFETY BELT AND LANYARD IN VIOLATION OF T-8CCR 1669(A).
