Employee injured in fall from I-beam

At about 8:40 a.m. on July 7, 1993, two employees were standing on an I-beam 8 in. wide and 8 ft high to lower the I-beam and weld boom onto the south end of the north bay. They were to use come-alongs (6 ton capacity chain hoists). When Employee #1 started using the ratchet handle of the come-along, it slipped through the sprocket wheel, causing the I-beam and weld boom to fall. Employee #1 fell with the beam and was trapped between the weld boom and a stationary column, suffering broken ribs on the upper back around the left shoulder. The come-alongs had been neither inspected nor maintained. Also, the employees were not tied with safety belts and lanyards. Employee #1 was transported to Kaiser Hospital, Fontana, where he was kept for observation and discharged the next day.
