Investigation Summary
An independent entrepreneur was setting up a mold on a Reed 300-ton molding machine at a plastics company. (He was assisted by his father.) He wanted the company to produce 439 automotive transmission filter covers after he set up the mold. The mold was presenting some problems, so he went up onto the machine to fix the problem. He slipped and fell onto exposed 480-volt band heater conductors. He was electrocuted.
