Investigation Summary
Employee #1, a tool grinder, was attempting to adjust the speed on a Brockhouse vertical mill. The mill had not been de-energized. While the employee was adjusting the pulley, his elbow actuated the start switch that energized the motor and actuated the pulley drive. Employee #1's right index and middle fingers were caught in the nip point of the belt and the pulley drive and were cut off at the first joint.
