Employee's fingers crushed between feed rollers

At approximately 12:30 p.m. on September 8, 1994, Employee #1, of West Coast Samples, Inc., was adjusting the blade after changing a knife on a Crawley paper cutter and shear machine. His left hand was on top of a piece of paper that was being cut when the feed rollers activated, pulling the paper and his hand between the rollers. Employee #1 was hospitalized with three crushed fingers on his left hand. Upon investigation, it was determined that the feed rollers were not disengaged after the employee had placed the control lever in the neutral position.
