Employee's fingertip amputated when caught in rollers

At approximately 11:30 p.m. on February 15, 1996, Employee #1 was making a splice-in at RC-1. The vinyl paper tail he was holding was pulled into the rolls, catching his finger in the closed nip point. The distal end of his right middle finger was amputated.
