Investigation Summary
At approximately 11:00 p.m. on August 8, 1992, Employee #1, of O.H. Kruse Grain & Milling, was working with a coworker to unplug an auger. A pipe wrench was placed on the shaft end of the auger. The pipe wrench, which was approximately 4 feet long, was left on the shaft as the coworker started the auger. The pipe wrench struck Employee #1's jaw; he required hospitalization.
