Employee's fingertip amputated by sprocket drive

On December 8, 1992, Employee #1 was performing preventative maintenance on an extruder, which extrudes plastic parts used in therapy of the heart and vessels. Employee #1 had not performed this assignment before. The equipment was still running when he wrapped an oil saturated rag around his left index finger and attempted to lubricate the chain and sprocket drive of the equipment. The oil saturated rag became caught in the drive and pulled Employee #1's finger into the drive, amputating the tip.
