Investigation Summary
At approximately 1:50 p.m. on December 8, 1992, Employee #1 was performing monthly maintenance on a Jensen folder/crossfolder in a laundry room at a Doubletree Hotel. Employee #1 was lubricating a chain and sprocket drive when his glove became caught, pulling his left hand into the sprocket. Employee #1's left little finger was amputated.
