Investigation Summary
On May 19, 1997, Employee #1 was operating a Chuo Kagaku Plastic Sheet Forming Machine Number 008, Model NHL-10-24, in the forming section of the production area. She was attempting to adjust the plastic sheeting being fed between the sheet feeder rollers. Her right hand was caught between the unguarded rollers. Employee #1 suffered serious injuries to her right hand.
