Employee's arm fractured in fall when scaffold plank tips

At approximately 4:10 p.m. on March 3, 1997, Employee #1 and two coworkers, of C.D.K. Contracting Company, were helping to rebuild the five stand section of a tin mill at a California steel facility in Fontana, CA. They were fitting a 20 ft by 4 in. pipe in the basement, working on and around a 12 ft section of scaffolding. Employee #1 climbed over the handrail of a stairway adjacent to the scaffold platform. He was stepping onto the scaffold when the planking tipped and he fell to the floor. Employee #1 fractured his right arm.
