Employee's arm injured in press brake machine

At approximately 10:03 a.m. on October 17, 1996, Employee #1 was working on an Atlantic Machine Company press brake machine, model #135X12HD, at RSR's steel fabrication site, Hesperia, CA. His right arm was fractured and he sustained crushing injuries when it became caught in the point of operation. Employee #1 had been trained by the employer and was familiar with the operation of the press brake. The cause of the accident was the lack of proper guarding on the machine. The employer was cited for a serious violation of T8CCR 4204(a).
