Investigation Summary
At 6:50 p.m. on April 21, 1999, Employee #1, a mechanic "A" employed by California Steel Industries, Inc., suffered a non-serious injury when the fatty pad of the tip of his right ring finger was cut off. Employee #1 used the side of a cut-off saw blade to remove a burr from the edge of the material he had just cut. He then reached between the machine's blade guard and the operating handle to access the ON/OFF switch to turn the saw off. The counter-balanced saw guard was in the down position. This exposed an area of the saw blade and arbor. Employee #1's gloved hand came close to the rotating saw blade and contact was made. This contact caused his gloved hand to be pulled into the exposed blade area. A sharp burr on the arbor cut his finger. Employee #1 was initially treated at the on-site medical clinic, and then taken to the San Bernardino County Medical Center for further treatment, and he was released and sent home. On April 23, 1999, an infection set in and Employee #1 went to St. Mary's Regional Medical Center in Apple Valley. The infection had progressed to a point that surgical amputation of the finger at the first joint was required. This procedure required him to be admitted to the hospital for an overnight stay, and changed the injury classification from non-serious to serious.
