Employee's fingers are amputated by machine

At about 5:15 p.m. on August 7, 2001, Employee #1 was operating a "Casa Herrera" baked chip maker machine when he tried to remove a glob of masa (dough) from the moving in-feed rollers of the sheeter head with his right hand. His hand inadvertently got caught in the rollers causing amputation of his right, middle, and ring fingers. The machine was not deenergized and stopped nor was an extension tool was used to removing the masa. Employee #1 was hospitalized for his injuries.
