Employee fractured foot under forklift frame

At approximately 10:30 a.m. on November 13, 2000, Employee #1, who was untrained and temporarily employed, was operating a forklift in the reverse direction at the dock gate Number 17 in the Receiving Department. The forklift was a "RAYMOND AND DOCKSTOCKER", model Number DSS300. When Employee #1 stepped off the moving forklift, the forklift swerved at the dock plate and his left foot was crushed under a forklift frame. Employee #1 was taken to Arrowhead Regional Medical Center, Colton, California for treatment. A surgery was performed and Employee #1 was hospitalized for five days.
