Investigation Summary
At approximately 8:30 p.m. on June 18, 2002, an employee was working with her supervisor on a cutter, Model Number 52C, Serial Number 1385501. The employee was sliding a piece of plastic film through the cutter when her right hand came into contact with the unguarded blade of the cutter. The supervisor did not notice that the employee's hand was in the point of operation when he started the cutter with the foot petal. The employee's right middle finger was amputated and she was hospitalized.
