Investigation Summary
Employee #1, a leadsman and mechanic, used a man lift to reach an 18 ft high conveyor dip belt system. He climbed from the manbasket onto an 8 in. channel iron welded to the two legs of the dip pulley structure. The manbasket was then lowered to the ground. While cutting a chain holding the dip pulley, the load broke a weld holding the channel iron on which Employee #1 was standing. He fell approximately 12 ft to where the man lift was stationed, his back striking the guardrail of the manbasket, and then fell another 6 ft to the ground. Employee #1 complained of back and neck pain and was taken to the hospital, where he was held for observation and pain management for 18 hours. He was then released to go home, but not to work. The cause of the accident was Employee #1's failure to tie off after the basket was lowered.
