Employee's fingers amputated by running press

At approximately 12:10 a.m. on August 16, 1994, Employee #1, the shift supervisor in charge of production for the graveyard shift for two months, was attempting to remove a jam from a running Lyle thermo-form press, serial #PM2465. He defeated the interlock safety device at the dye press door by putting a bolt on it. When he reached into the running dye station (trim press station), his left hand was crushed by the downward stroke of the trim press and his little and ring fingers were amputated. He was taken to Loma Linda University Medical Center for treatment. A serious citation of T8CCR 3314(a) was issued to the employer.
