Employee's head injured when struck by falling angle iron

At or about 11:00 a.m. on January 4, 1994, Employee #1 was cleaning the metal pieces from a site when a 23 ft 9 in. long piece of angle iron (6 in. by 4 in. by 3/8 or 1/2 in. thick) that had been cut on the previous day by another employee at the west end of the site came loose at the east end bolt and fell 30 ft, hitting him on the head and legs. He was wearing a hard hat, but suffered a head injury, a broken right thigh, and a laceration of the left thigh. He was hospitalized for several days at Loma Linda Medical Center. The angle iron was part of the structure being dismantled in the NE of the skull cracker area. No one was working on the upper level. Employee #1 and the team leader did not know of the previous day's cut. No violation was cited.
