Employee injured when toes crushed by fork lift tire

At 3:20 p.m. on March 27, 2001, an injury accident occurred in the material receiving department of Contractors Wardrobe, Inc., a home furnishing products manufacturing facility. While loading crated and boxed glass panels onto a fork lift truck, the vehicle moved and the left front tire rolled onto the toes of Employee #1 who was attempting to straighten the load on the lifting forks. The fork lift driver secured the vehicle and assisted Employee #1 who was then transported by ambulance to Henry Mayo Hospital. He was hospitalized until March 30, 2001, with a dislocated large toe and various lacerations.
