Investigation Summary
On April 12, 1999, an employee was climbing down an aluminum A-Frame ladder while carrying a box of material weighing approximately 10 lb. The employee slipped down the ladder and fell over an adjoining partition that was approximately 8-ft 6-in.-tall. The employee then fell through an acoustical tile office ceiling and landed straddling a 4-ft-high divider frame that was around an office cubicle. The employee suffered severe, painful injuries to his groin area and was hospitalized.
