Investigation Summary
On August 12, 1997, Employee #1, a machine operator, was inside a box-manufacturing machine attempting to adjust the operation. This adjustment is a routine procedure. The machine has two major portions. One portion is stationary and the other portion is on a track. The part that is on a track was rolled out to the adjustment position to allow access for adjustment. While Employee #1 was performing the adjustment, another employee driving a forklift collided with the track- mounted portion of the machine forcing it to close on Employee #1. Employee #1 suffered a serious crushing injury and was transported to the hospital.
