Investigation Summary
Employee #1, a mechanic for Los Angeles County Sanitation, locked out the ON and OFF switch at the pump drive in order to change the drive belts. He was unaware that this pump served as a backup for another use. While Employee #1 was working on the drive, the operator in the control room started the pump from the control panel. The tip of Employee #1's ring finger was amputated at the first joint. It was reattached and the prognosis was excellent for full restoration of use of the finger.
