Investigation Summary
On January 22, 2014, Employee #1 was making hamburger patties on a Hollymatic Pattie machine which holds approximately 60 to 80 pounds of meat. The guard over the feed chute was broken off and he was pushing the remainder of the product down the chute when his finger was pulled into the rollers. Employee #1 was taken to the hospital where his left middle finger was amputated to the first knuckle.
