Employee's Finger Is Caught in Conveyor and Is Amputated

On September 11, 2011, Employee #1, the manager, was placing a chain onto a sprocket of a conveyor. As he worked, his left middle finger was caught between the sprocket and chain, resulting in its amputation. Employee #1 was transported to medical center, where he underwent surgical procedures and was hospitalized for four hours.
