Investigation Summary
At about 11:00 a.m. on March 13, 1994, Employee #1 was weighing boxes of spinach. The production process was going smoothly, so she decided to clean up in her area; cleanup is encouraged by the supervisors. As she was cleaning around the PRC machine, which boxes the spinach, her rubber glove with liners came into contact with the chain conveyor tightening sprocket. This sprocket is under the chain conveyor. Her fingers were drawn into the chain and sprocket and the right index finger was amputated. The ring and middle fingers were also damaged. She was hospitalized for 8 days.
