Employee's fingers lacerated in loading mechanism

At approximately 10:30 a.m. on October 12,1994, Employee #1, a temporary employee, was operating a 3M-Matic (2009) adjustable case sealer with Accuglide II taping heads, type 29200 by 3M Co. Employee #1 was trying to adjust the top tape in the loading mechanism while the machine was running and the conveyor was moving boxes for taping. One of the boxes trapped Employee #1's hand under the moving top taping head and the knife blade for paper cutting. The first and second fingers on his left hand were cut. Employee #1 was taken by ambulance to St. John's Regional Medical Center in Oxnard. He was released after treatment for finger laceration. Citations were issued: 342(a), 3203(a)(7)(c), and 3384(a).
