Employee killed when crushed by overturned forklift

At 9:41 p.m. on February 18, 1995, Employee #1, of Proctor and Gamble Corp., was using a Hyster forklift truck, co. #595, serial #A187V08146 H, to pick up an approximately 4,377 lb bundle of eight bales of raw pulp paper. After he picked up the bundle, he raised it to the top of the mast. Apparently, when Employee #1 tried to make a reverse right-hand turn, the forklift truck overturned and fell onto its left side, crushing him. Employee #1 suffered multiple internal blunt force trauma injuries and died. His body was taken to the Ventura County Morgue.
