Investigation Summary
At about 2:25 a.m. on June 6, 1994, Employee #1, a supervisor for Pictsweet Mushroom farms, was on a work platform that was being elevated by the forks of a forklift truck. Employee #1 walked from the right to the left side of the work platform. The work platform fell off of the forks of the forklift truck. Employee #1 fell about 7 feet to the asphalt surface. Employee #1 was treated by a private physician and he was released to return to work.
