EMPLOYEE INJURED BY SHATTERED BUFFING MANDREL

At 1:16 p.m. on March 21, 1994, Employee #1 was buffing a mandrel made from thermoplastic polyester ertalyte. He was using a handle to manipulate the mandrel during this operation. The mandrel shattered, injuring Employee #1's face and splitting the Sellstrom brand face shield he used for protection. Employee #1 had lacerated both of his lips, had dental fractures and a nose cut. The injured employee was moved to Goleta Valley Community Hospital.
