Investigation Summary
At approximately 2:30 p.m. on September 17, 1995, Employee #1 was backing a Hyster electric forklift #574 into a battery stall. His right hand was outside the driver's compartment, wrapped around the fall cage bar. His fingers were squeezed against a pole located alongside the parking area. Employee #1 was treated at St. John's emergency room and then released. A general citation was issued.
