Employee's hand lacerated when caught in lowering mast

At approximately 4:45 p.m. on October 24, 1995, Employee #1 and two coworkers were trying to close an overhead receiving door that was stuck in an elevated position. To reach the door's bottom section, Employee #1 stood on the tines of a forklift. The forklift operator was lowering the mast assembly when Employee #1's right hand became caught in the mast components. He sustained lacerations that required hospitalization. The employer was cited for a serious violation of T8CCR 3664(a)(5) because Employee #1 placed his hand into an area where a shear or crushing hazard existed; and for a violation of T8CCR 3657(a)(1) because the forklift was not equipped with an appropriate platform for elevating employees.
