Employee's foot crushed when caught in wire mesh rollers

At approximately 10:30 a.m. on October 21, 1995, Employee #1 and two coworkers were in the northwest corner of the wire room, guiding wire fabric mesh into the north rollers. Another coworker was operating the rollers from the front, rolling the wire mesh into cylindrical cages for use in manufacturing pipes. Employee #1 and a coworker were standing on the wire mesh at the rear of the rollers, pushing it into the rollers with their feet. Employee #1's left foot became caught in the rollers and was crushed up to his ankle. He was taken to San Bernardino County (CA) Medical Center, where his foot was amputated above the ankle. The in-running side of the rollers was not protected by a fixed or self-adjusting guard, or the equivalent, to prevent workers from being caught in the nip point at the rollers. The employer was cited for a serious violation of T8CCR 4187(a).
