Employee injured in fall from aerial lift

At approximately 8:30 a.m. on November 25, 1994, Employee #1 and a coworker, of Hahn Property Management, were placing Christmas decorations on the second floor railing at the Palm Desert Town Center, Palm Desert, CA. Employee #1 was using a Genie lift that was extended to the second floor level. The lift was equipped with attachments for outriggers, but they were not being used. The coworker was repositioning the lift for Employee #1 when a wheel of the lift would not turn because of a depression between the ceramic tiles of the floor. The coworker may have pushed the lift sideways to get the wheel over the depression, causing Employee #1 to lose his balance and the lift to tip over because of the weight shift at the top of the lift. Employee #1 fell approximately 15 to 20 ft to the tile floor and sustained fractures and lacerations.
