EMPLOYEE IS STRUCK BY A MAST

At approximately 10:27 a.m. on April 19, 1994, Employee #1 and two coworkers were installing a mast assembly on a Hystor H300 forklift. To install the mast, another forklift was used to elevate the 8000 lb mast assembly and push the mast into its respective attachment point of the Hyster 300B forklift. As pressure was being applied by the lifting forklift, a wood block slipped resulting in the mast being installed striking Employee #1 in the head. The wood block measured four inches by four inches by twenty-four feet. Employee #1 was hospitalized for seven days. The employer was cited for a serious violation of Section T-8CCR 3664(e).
