Investigation Summary
On February 7, 2001, Employee #1, Slitter Operator, for NCR, was attempting to unjam a chain on a tray leading from a slitter machine. He grabbed the chain and pulled on it, it start to move and his thumb got caught between the chain and the drive sprocket. He suffered amputation of his left thumb at the first knuckle. He was hospitalized.
