Investigation Summary
Employee #1 was operating a box forming machine. One portion of a box was jammed in the machine. Employee #1 shut off the machine and put a lock pin in the mandrel to prevent it from lowering. While reaching in to remove the box, he rested his hand on the top guard of a chain sprocket drive. The drive, however, was still running and his pinky on his right hand got caught in the unguarded side of the sprocket drive. As a result, a small portion of his pinky was amputated.
