EMPLOYEE #1 WAS WALKING BACKWARD ON A FLOOR OR ROOF WHILE USING A DEVICE TO PUNCH HOLES IN METAL DECKING IN ORDER TO INSERT RODS TO HANG INSULATION. HE STEPPED THROUGH AN OPENING AND FELL 15 FEET TO EXPOSED REINFORCING STEEL. HE SUFFERED A PUNCTURE WOUND TO HIS CHEST AND A FRACTURED RIGHT WRIST.
