Employee catches fingers between rollers

At about 8:00 a.m. on September 18, 1993, Employee #1 caught his right hand in the in-running rollers on web #1. Employee #1 was applying ink on the rollers with a spatula when his hand slipped and went in between the #3 form roller and the copper accelerating roller. His fingers were crushed and he was taken to Tustin Medical Center for treatment and hospitalization.The in-running ink rollers were not guarded, violating t8 ccr 4441. A serious violation is being issued for failure to guard the in-running ink rollers.
