Two lab employees overexposed to irritating vapors

At approximately 8:15 a.m. on June 3, 1993, a floor drain in the radiology lab became plugged. An unauthorized employee put drain cleaner in the drain, which resulted in the release of irritating vapors throughout the lab. The facility was evacuated and the problem was resolved. Based on descriptions of the odors by the maintenance employee and a written report of the incident by the lab, one of the vapors may have been hydrogen sulfide. Employees #1 and #2 were hospitalized for four days for observation and treatment. A regulatory general citation was issued for not reporting a serious injury within 24 hrs (342A). A general violation was issued relative to MSDS-5194(g)(8) and a serious violation was issued relative to Training- 94(h)(1).
