Employee's fingers amputated by sprocket and chain drive

At about 9:35 a.m. on June 29, 1993, Employee #1, of Orchid Paper Products, was assigned as a tender on a facial machine. Paper was wrapped around the sprocket and chain drive so he tried to clear it while the machine was running. His left index finger was caught between the sprocket and chain and amputated between the first and second knuckle. He was taken by paramedics to La Palma Hospital for treatment. Employee #1 said that he had used an extension tool to remove the jammed paper, but the tip of the tool got caught in the chain opening and his hand was pulled into the sprocket and chain drive. He said he knew that he was supposed to notify the machine operator to stop the machine before clearing but did not do so because he did not want to stop production. Employee #1 received and signed the safety booklet that requires stopping the machine before clearing it. There is no violation that would have related to the accident.
