Employee suffers amputation of fingers between belt/pulley

Employee #1, a maintenance mechanic, was installing a belt on a pulley without releasing the tension on the pulley. He caught three fingertips of his right hand between the belt and the pulley. Two fingers were amputated at mid-fingernail.
