Investigation Summary
At approximately 11:45 a.m. on January 25, 1993, Employee #1 was working underneath the falsework during a concrete pour. The falsework collapsed and approximately 250 cubic yards of concrete, rebar, and shoring material fell to the lower level, crushing Employee #1. The 34 ft high falsework had not been inspected by a registered civil engineer either after it was constructed or before the placement of live loads. The employee was extricated by local fire rescue and was pronounced dead at the scene by the coroner.
