Investigation Summary

Investigation Nr: 170167159
Event: 10/30/1992
Employee injures leg in collision of two forklifts

On October 30, 1992, Employee #1 was operating a standup forklift in the distribution area of a warehouse. Employee #1 was driving backward toward the work area, in accordance with company policy. He then collided with a coworker who was driving a sit-down forklift down the dock. Employee #1 had seen the coworker and lifted his leg off the deadman brake pedal. Instead of lifting his leg straight up, he bent his knee with his leg extending behind him. The other forklift hit his leg, pushing his knee into the front of the machine. Employee #1 required hospitalization.
