Employee's finger amputated by dado blade of table saw

At approximately 8:00 p.m. on August 26, 1992, Employee #1 was using a Delta table saw with a double saw blade to dado the ends of one side of 150 pieces of wood 3/4 in. thick by 7 in. wide by 37 in. long. There was no hood guard for the saw. When Employee #1 was working on the 51st piece, he accidentally fed the wood along the rotating blade. The wood was ejected and his right index finger contacted the saw and was amputated. He was taken to Golden West Medical Center, Anaheim, for treatment and then transferred to Anaheim Memorial Hospital. The cause of the accident was the absence of a hood guard, which could have prevented the employee from feeding the wood on the wrong end. A serious citation violation of 8CCR 4300(a) was issued.
