Investigation Summary
At 10:00 a.m. on July 19, 1995, an employee arrived at work and went to the administrative offices, where he shot and killed Employees #1 and #2. He then went downstairs to a workshop, where he shot and killed Employees #3 and #4. The employee, a communications electrician, was being considered for a conduct review hearing due to his poor productivity and inability to work with others, including his potentially violent behavior. Employees #1 through #4 were supervisors who had been writing performance evaluation reviews of the employee. The incident took place at the City of Los Angeles Department of General Services, Communications Services Division, in the C. Erwin Piper Technical Center. The city's formal disciplinary procedures for immediate management action were not implemented and the employees had not received training on procedures for workplace violence prevention.
