Employee's finger amputated when he slips and falls

On March 7, 1995, Employee #1, working for a manufacturer of pressure vessels, was doing a final check of a recently installed alternator belt on a forklift truck. He was approaching the truck when he slipped in water and grease on the concrete floor and fell forward. While he was attempting to stop his fall with his hands, his left hand slipped and entered the area of the alternator belt. His ring finger was injured and his little finger was amputated from the second knuckle. The floor had no grates, platforms, or drainage to control accumulated water. According to management and employees, accumulated water was a common occurrence during the rainy season.
